Course Description

This course focuses on the clinical experience and application of nursing care management for adult patients with multi-system and emergent health problems in a variety of settings. Concept-based curriculum provides the structure of the curriculum and is threaded throughout the course. PREREQUISITE: NURS G175, NURS G175C, NURS G285, and NURS G285C. COREQUISITE: NURS G290. ADVISORY: NURS G271. Transfer Credit: CSU.

Course Level Student Learning Outcome(s)

1. Apply the nursing process to provide safe evidence-based care to critically ill adult patients with multi-system and emergent health problems that adheres to legal, ethical, and professional standards.
2. Integrate the understanding of pathophysiology of common acute/chronic health problems to the care of critically ill patients.
3. Use information technology in collaboration with nursing and healthcare teams to facilitate communication and to coordinate patient care.
4. Develop a patient-centered collaborative plan of care for critically ill adult patients with multi-system and emergent health problems.

Course Objectives

• Assemble assessment data that includes physical, psychological, developmental, sociocultural and physiological functioning on diverse patients with multi-system, critically-ill, emergent health conditions.
• Analyze clinically significant assessment data pertinent to patient's priority problems.
• Develop evidence-based nursing care plan for adult patients with multi-system, critically-ill, emergent\\nhealth problems that adheres to legal, ethical, and professional standards.
• Integrate the clinical judgment measurement model to meet the healthcare needs of critically-ill\\npatients.
• Utilize interdisciplinary collaboration teams during a patient emergency.
• Manage medical emergency to promote positive patient outcomes.\\n
• Use informatics and technology, including electronic health records, to collect and document patient care for critically ill patients.\\n
• Detect potential multi-system complications that may emerge from comorbidities and critical health problems.\\n
• Evaluate patient assessment data to establish priority nursing care.\\n
• Revise plan of care based on evaluation of patient outcomes.

Lab Content

Patient-centered Care Utilizing the nursing process to provide compassionate, culturally sensitive care that is based on the physiological, psychological, sociological, spiritual, and cultural needs, preferences, and values of the patient. Safety and Quality Improvement (QI) The minimization of risk factors that could cause harm whil promoting quality care and maintaining a secure environment for patients, self and others in order to improve health care services and better meet the needs of patients. Nursing Judgment/Evidence Based Practice (EBP) The use of current knowledge from researc and other credible sources, in consideration of the nurse s clinical expertise and patient preferences, to make nursing clinical judgments and provide patient, family and community centered care. Teamwork and Collaboration The delivery of patient care in partnership with nursing and interdisciplinary teams to achieve continuity of care and promote patient outcomes. Informatics and Technology The use of information and technology as a communication and dat gathering tool that supports clinical decision making and safe, scientifically based nursing practice. Professional Identity The adherence to legal, ethical, and professional standards of practice to provide nursing care for patients across the lifespan. For the following concepts, apply the nursing process and collaborative management for improved patient outcomes focusing on adult, geriatric and mental health patients. Cognition: Thinking skills including language use calculations perception memory awareness reasoning judgment learning intellect social skills and imagination. Implement nursing assessment to iden tify patients at risk/with cognitive deficits. Implement nursing interventions to care for patients with cognitive deficits. Collaborate with health care team for management of geriatric patients with cognitive deficits. Fluid & Electrolytes/Acid/base: The physiological mechanisms that maintain fluid and electrolyte balance. Collaborate with health care team for management of patients with acute, complex alterations in fluid and electrolyte balance. Implement nursing care of patient with chronic disease experiencing complex alterations in fluid and electrolytes/acid base balance. Grief and Loss: A series of responses that occur following a physical and/or psychological insult with a goal of returning to homeostasis Implement nursing care of families experiencing a situational loss. Immunity: The body's protective response to infection and disease. Collaborate with health care team for management of adult/geriatric patient with severe alteration in protective response to infection. Implement nursing interventions to minimize complications of altered immune response. Infection: Infection or infectious disease is a state of tissue destruction resulting from invasion of microorganisms into the body. Collaborate with health care team for management of adult/geriatric patients with multi-system infections. Implement nursing interventions to minimize complications of severe infections. Metabolism: All biochemical processes that occur within the body. Collaborate with health care team for management of adult/geriatric patient with chronic alternations in metabolism Provide individualized teaching regarding chronic alterations in metabolism. Mood/Aff ect: Mechanisms that influence the emotional state of an individual and its outward manifestations. Collaborate with health care team for emergency management of adult/geriatric patients with severe mood/affect disorders. Provide evidence-based care for patients with chronic mood/affect alterations. Nutrition: The process by which the body ingests absorbs transports and uses nutrients and food. Collaborate with health care team for management of patients with severe nutritional deficits. Provide evidence-based care for patients with severe nutritional deficits. Oxygenation: The exchange of gases (oxygen and carbon dioxide) in cells. Collaborate with health care team for management of patients with severely compromised oxygenation Provide evidence-based nursing interventions for patients with acute oxygenation compromise. Perfusion: The process of fluid nutritive and oxygen delivery to cells. Collaborate with health care team for management of patients with acute, life-threatening alterations in perfusion. Provide evidence-based nursing interventions to minimize complications of diseases that compromise perfusion. Tissue Integrity: Mechanisms that facilitate intact skin and mucous membranes and their physiological functioning. Collaborate with health care team for management of patients with severe destruction of skin integrity. Provide evidence-based nursing interventions to minimize complications of major skin integrity problems.
